How would you deal with this in the USA? An 18 year old man comes on holiday with his 16 year old wife?

I would turn up the hotel tv and ignore them.Your best bet is to just carry on in your own life and ignore others.we americans all have this busy body syndrome where we have to be up in peoples business all the time...I ignore people,it's their business.
